What This Form Is For:

This online form should be used to submit donations of merchandise, gift certificates, and approved underwriting for Valle Monte League’s 2025 fundraising events.

Form Instructions:

Fields marked with an asterisk (*) are required.

• Item:
  Please submit each donation item separately. Do not group multiple items into a single entry—unless they are always intended to be kept together.

• Estimated Value:
  Provide your best estimate of the item’s value.

• Section:
  Indicate where the item is intended to be used—Tree, Silent Auction, Live Auction, or Other. You may also select "No Preference" and the Gifts Committee will determine the best fit.

• Location:
  Specify the current physical location of the donation item.

• Soliciting VML Member:
  Enter the name of the Valle Monte League member who secured the donation. This may be the same as the donor or a different person.

• Contact Information:
  Enter the donor’s name and contact details. This is the information that will be used for all recognition purposes.
  ⚠️ If you are submitting the form on behalf of someone else, please do not enter your own contact information.
